Declaration by president and secretary confirming the correctness of information
Resolution of the first governing body meeting approving formation and registration
Covering letter signed by founding members submitting documents to Registrar of Societies
Affidavit in the prescribed form in Rs.100/- E Stamp paper duly notarized
